换领枫叶卡 DIY

换领枫叶卡的条件:

你在5年中住够2年（730天）
你本人在加拿大
不是加国公民
没有被加拿大政府驱逐出境

填写表格:
首先要填写的表格是IMM5444 (这个表格相当重要，表格可以从http://www.cic.gc.ca/english/information/applications/prcard.asp下载下来在电脑里填写，也可以打印出来手填)

下面按表格上的号码依次排序：

语言（一般在英语方框中打钩）
你成为加拿大永久居民的时间（在你枫叶卡的背面有写）
枫叶卡申请：第一张枫叶卡；换领枫叶卡；丢失，销毁或从没拿到枫叶卡（这 3个框中，你要选择一个）
Note：当你换领枫叶卡的时候，你需要保存好你现在的枫叶卡，当你的新枫叶卡寄回给你的时候，你会被要求销毁你的旧卡。如果你的卡损坏了，那么你必须把损坏的卡和你的申请表一起寄回加拿大政府。
你的全名（和枫叶卡，移民纸上的一样，要填写姓氏及名字）
你现在的名字（这个是给刚改过人名的人使用的，当然你要附上你的改名证书）
性别
出生日期
出生国家
现在的国籍（如果你现在是中国国籍，那么你就需要填写Chinese）
眼睛的颜色（这个在你枫叶卡的背面也有写，一般都是brown，当然也有例外啦）
你的身高（填cm这个就可以了）
你目前在加拿大的居住地
你目前在加拿大的邮寄地址（如果和你的居住地址一样就不需要填写了，填写N/A就可以了）
你的电话号码及邮箱
你的婚姻状态（从未结婚，结婚，丧偶，分局，离婚，同居）
你的登陆地点（你要写上是哪个城市，哪个省份）
你有过被加拿大政府驱逐的历史么？
你有没有被移民官告知你不满足加拿大的居住义务？
居住历史（过去5年所有居住历史，包括加拿大及海外的（短期的就不算了））
工作/教育记录(过去5年的工作机教育记录)
旅行记录（过去5年所有在加拿大以外的居住记录，如果是美国当天往返的不算，其他都算，全部要写出离境天数及日期）（如果这5年当中你都没有离境的话，就请在No的地方打钩）
如果你在加拿大以外的国家居住时间超过1095天，也就是超过3年了，那么你就需要填写22项的Option1,2,3。
Option 1. Absence while accompanying a family member who is a Canadian citizen 你在加拿大以外居住的这段期间是否有一个有公民身份的人陪伴（一般是指配偶，同居关系，未成年人的父母）
首先你要填写他的名字，和你的关系，其次，你要写清楚国外的居住日期，地点及离境天数。
Option 2. Absence while employed by a Canadian business or in the Canadian federal or provincial public service
是否被一个加拿大本地公司派往外地办公(如果是这类申请人的话，应当提供很多与公司的关系证明，如雇佣合同，工资单等）
你要填写离境日期，地址及离境天数
Option 3. Absence while accompanying a family member who is a permanent resident of Canada and who is employed by a Canadian business or the Canadian federal or Provincial public service
在陪同你境外居住的家庭成员是否被一个加拿大本地公司所雇佣（是上述2个问题的补充）
你要填写陪同人姓名，关系，及离境日期，地址，天数。
*以上这些表格不够用的都可以自己打印出一个文档来附在申请表格的后面
History of entries to Canada (这段的意思是说你要不要授权移民局可以从边境管理处调你的个人档案，如果填Yes的话就是可以，填No,的话就是说你也许被要求要自己去调档。)

丢失及补办枫叶卡

Complete this section only if you are applying to replace a lost, stolen,destryed or never received PR Card. 完成这个部分当你的枫叶卡丢失，被偷，销毁或者从来没有收到枫叶卡。

Photocopy of one (1) primary identity document. See section “Step 1. Gather Documents” in the instruction guide.

Photocopy of one (1) secondary identity document.

Two (2) photos placed in small envelope
(no staples or paper clips). Photo must follow the Photograph Specifications for Permanent Resident Card.35*45mm的两张照片，要装在一个小纸袋里（枫叶卡照片，照相馆都知道）
Fees. Copy 2 of the Receipt form showing the amount paid. See section “Pay the fees” in the instruction guide for the acceptable methods of payment.（如果在网上交钱的话，需要打印2张费用收据）

You are under the age of 18: a photocopy of your birth certificate, adoption order or legal guardianship document and school records. See section “Step 1. Gather Documents” in the instruction guide.
如小于18岁，出生证明，领养，监护认证明和学校成绩单则需准备
Proof of residency requirements:
If you were outside Canada for 1095 days or more in the past five(5) years.
